Tutorial: Building & Deploying Custom Checklists

Step-by-step guide for managers to design dynamic checklists, configure field validation, and bind forms to locations

In this tutorial, you will learn how to build a custom opening checklist form from scratch, add text, rating, and temperature number fields, bind the form to a specific restaurant location, and verify that submissions update compliance and Slack channels.

Time to complete: 10 minutes


Step 1: Access the Form Builder

Only team administrators can access the form building canvas.

  1. Navigate to Admin → Forms in the sidebar.
  2. Click Create New Form in the top right.
  3. In the metadata modal, set the general properties:
    • Form Name: "Morning Operations & Safety Audit"
    • Description: "Daily pre-opening walkthrough to check walk-in temperatures, clean stations, and verify POS status."
    • Status: Keep as Draft for now.
  4. Click Next: Field Builder.

Step 2: Build Your Fields

We will add three fields to test different validation capabilities.

Field A: Walk-In Cooler Temp (Number Validation)

  1. Click Add Field at the bottom of the canvas.
  2. Set the properties:
    • Label: "Walk-In Cooler Temperature"
    • Field Type: Select Number.
    • Required: Toggle to Yes.
    • Placeholder: "e.g. 36"
  3. Expand Validation Rules:
    • Minimum Value: Set to 32.
    • Maximum Value: Set to 40.
    • Custom Error Message: "Cooler temp must be between 32°F and 40°F."

Field B: Kitchen Line Cleanliness (Rating Scale)

  1. Click Add Field.
  2. Set the properties:
    • Label: "Prep Line Sanitation Score"
    • Field Type: Select Select (Dropdown).
    • Required: Toggle to Yes.
  3. Under Dropdown Options, add three options:
    • 1 — "Unsatisfactory (Re-clean required)"
    • 2 — "Satisfactory (Meets standard)"
    • 3 — "Exceptional (Spotless)"

Field C: Verification Signature & Notes (Text Area)

  1. Click Add Field.
  2. Set the properties:
    • Label: "MOD Verification Notes"
    • Field Type: Select Text Area.
    • Required: Toggle to No (optional note-taking).
    • Placeholder: "Document any equipment issues or corrective actions here."

Step 3: Location Binding & Slack Routing

Forms can be localized so that staff only see forms relevant to their immediate restaurant location.

  1. Click Next: Routing & Deploy.
  2. In the Location Assignment section:
    • Select the checkboxes for the specific locations (e.g. "Downtown" and "North Side") where this opening checklist is active. Leave other locations unchecked.
  3. In the Slack Routing section:
    • Mapped Slack channels will receive block-formatted digests.
    • Set Default Channel: Select #ops-alerts or enter the channel ID.
  4. Click Publish Form.

Step 4: Verify as a Staff User

Let's test the published form to verify autosave and validation.

  1. Click the location switcher in the top nav and select "Downtown".
  2. Navigate to Forms in the main sidebar. You should now see "Morning Operations & Safety Audit" listed.
  3. Open the form.
  4. Test Autosave:
    • Select a cleanliness score.
    • Close the browser tab.
    • Reopen the form link. Note that the cleanliness rating was restored from your local draft.
  5. Test Validation:
    • Enter 45 in the Walk-In Cooler Temp field.
    • Tap Submit.
    • Note the Zod validation error: "Cooler temp must be between 32°F and 40°F."
  6. Enter 36 (valid) and click Submit.
  7. Observe the success state and verify the #ops-alerts channel receives the automated Slack Block Kit card.
