Fill out and submit forms for your location
Overview
Forms are used to collect important information from staff — checklists, incident reports, shift logs, and more. Your manager configures which forms are available for your location.
Accessing Forms
Forms are available from the main navigation under Forms. You will only see forms that are active and assigned to your location.
Filling Out a Form
- Open the form from the Forms page
- Complete all required fields (marked with *)
- Upload images if the form includes photo fields
- Submit the form when finished
Field Types

Draft Autosave
Your work is saved automatically as you type. If you close the browser or lose connection, your draft will be restored when you return. Drafts are stored in your browser and are unique to your account.
Uploading Images
Some forms allow photo uploads. Supported formats:
- JPEG / PNG
- HEIC (iPhone photos are converted automatically)
Images are uploaded securely and appear in your submission and any Slack notifications sent to management.
After Submission
- A confirmation message appears on screen
- Management receives a Slack notification with your responses
- If the form is part of compliance tracking (e.g., opening checklist), your compliance status is updated automatically
CSV Export
Admins can export all form submissions as a CSV file for analysis in spreadsheet tools.
